From the first day a resident is admitted to Boulevard Temple,
on-site physicians, nurses, and rehabilitation specialists begin
developing and implementing an individual plan of treatment.
To assure optimum results for every resident, our health care
team includes:
- Physical therapists, occupational therapists and speech
language pathologists
- Board-certified physiatrist--physician specializing in
rehabilitation
- On-site physicians from nearby medical centers
- A medically trained admissions director to coordinate
all referrals and admissions
- Compassionate, highly skilled nurses, nursing assistants,
social workers, and dieticians

Hospitals provide excellent care during the acute stage of an
illness or injury. But they don’t always prepare patients
to return home. What’s needed is a period of time when
individuals receive expert, coordinated care from a team of
rehabilitation and nursing specialists.
Boulevard Temple offers a unique Hospital to Home program designed
to help residents regain function, rebuild strength, relearn
skills, and develop the confidence they need to return home
and lead more independent lives.
By combining the skills of certified therapists and physicians
with an attentive nursing staff, a supportive and comfortable
environment, a faith-based philosophy of care, and a focus on
follow-up and support services, we assure that every individual
will make the most complete recovery possible.
At every phase of the healing process, we strive to provide
the highest level of care and service. To help assure a successful
outcome, staff members offer one-on-one health education for
residents and family members, as well as customized home exercise
programs to help maintain functionality.
To ease the return home, we can connect residents with a network
of community resources and support services. We can also arrange
for a comprehensive home evaluation and provide practical suggestions
for creating a comfortable, easy-to-navigate environment. Upon
request, we can even help locate a new home setting—one
that better meets the needs of the resident.
